When every miss looks the same in a tracker, owners treat them the same — which usually means slow closure and quiet normalization of drift. Triage is the discipline of separating signal types before you assign work.

Three buckets that change behavior

Transient misses are explainable one-offs with intact compensating evidence. Process friction shows repeated near-misses tied to capacity or tooling. Policy drift appears when the written rule no longer matches how revenue, payments, or approvals actually move.

Labeling is not cosmetic. It decides who owns the fix: a single operator, a process owner, or a policy steward who must reopen governance.

A weekly filter that takes twenty minutes

Sort new exceptions by recurrence across entities, dollar or volume exposure, and age since first detection. Anything recurring across two periods graduates out of “transient” automatically — no debate in the stand-up.
